Sport Physicals
All student-athletes are required to
have a sport's physical on file to participate in any school-sponsored
sport. The physical form must be filled out by a doctor along with
insurance information and parent signature. All physicals must be
updated each school year. Below you will find a copy of the physical
form along with information from the Convenient Care Clinic. Please
