KMS Athletics

At Lake Shore, we believe a balance of academics and athletics creates a well-rounded student.  Students who participate in sports not only gain the benefits of living a healthy lifestyle, they also learn lessons about sportsmanship, leadership, teamwork, cooperation, respect, and knowing how to win and lose.

Remember game times may change due to conflict.  You can always call (586) 285-8810 to confirm game times.  Thank you!

Sport Physicals

All student athletes are required to have a Sport's Physical and Emergency Card on file to participate in any school sponsored sport.  Students bring in their completed physical and emergency card to Mrs. Cavazos, located in the back office, to receive a green card. The green card is needed before a student can participate in any practices or tryouts. Students will present their green card to the coach at their first practice.

The Physical Form must be filled out by a doctor and include insurance information and a parent signature.  This year, due to the Covid pandemic, any 7th and 8th grade student that has a physical on file from last school year, 2019-2020, will not need a new physical. Instead, a 2020-2021 MHSAA Sports Health Questionnaire will need to be completed and turned in.  If Yes is answered to any of the questions on the Questionnaire, then a new physical will be required.

Emergency Cards can also be used from 2019-2020. If you need to update any information, please fill out a new Emergency Card.

We have physical forms, emergency cards and health questionnaires here at KMS that can be picked up in the back office. You can also locate the forms on our website by clicking the Sports Forms link on the first page.
